Forbes College Department

Incoming First Year Student Requirements

  • Original Copy of Form 138/ Report Card
  • PSA Birth Certificate (1 Original Copy or Certified True Copy)
  • Passport-sized colored photo with white background (2 Copies);
  • Original Certificate of Good Character

Transferees

  • Honorable Dismissal (HD) or Certificate of Eligibility
  • PSA Birth Certificate (1 Original Copy or Certified True Copy)
  • Certificate of Grades or Photocopy of the Transcript of Records
  • Passport-sized colored photo with white background (2 Copies)
  • Original Certificate of Good Character

Returnee

  • Secure clearance from the Registrar
  • Get an updated evaluation of grades from the Adviser/Program Chair; and
  • Submit copy of Marriage Certificate (if applicable).

Shifter

  • Evaluation of grades from the Admitting Department/Program Chair

Senior High School

Requirements for Incoming Grade 11 Students

  • 2 - 2x2 Colored ID Picture white background;
  • Photocopy of PSA Birth Certificate
  • Original and photocopy of Form 137 from previous Junior High school attended
  • Original copy and photocopy of Form 138 (Report Card) of Grade 10 signed by the principal
  • Certificate of Good Character from the previous school attended
  • Photocopy of Diploma (Certificate of Completion)

Transferees

  • Transcript of Records/ Certified True Copy of Grades
  • Certificate of Honorable Dismissal
  • Photocopy of PSA Birth Certificate (3 copies)
  • 2 copies - 2x2 Colored ID Picture white background
  • Certificate of Good Character (Original).
