ADMISSION REQUIREMENTS:

To become an official member of Forbes College, first, you have to take the qualifying examinations. After the exams you can now enroll. Just bring the following documents:

FRESHMEN:

  1. High School Report Card (Form 138) – original copy
  2. Certificate of Good Moral Character – original copy
  3. Photocopy of NSO Birth Certificate
  4. Recent 2 x 2 I.D. pictures (2 copies)

TRANSFEREE STUDENTS:

  1. Certificate of Transfer (Honorable Dismissal)
  2. Certificate of True Copy of Grades (T.O.R.) – for evaluation purposes
  3. Certificate of Good Moral Character
  4. Photocopy of Birth Certificate
  5. Recent 2 x 2 I.D. pictures (2 copies)

 

 

ADMISSION REQUIREMENTS TO FORBES ACADEMY:

Pre-school

  1. The child must be 4 ½ to 5 years of age
  2. Must have passed the interview
  3. Must submit a copy of NSO Birth Certificate

Elementary (Grade 1)

  1. Must have attended Pre-school
  2. Must not be below six (6) years old at the time of enrolment
  3. Must submit report card duly signed by the Principal/Director, NSO Birth Certificate, 2 copies of recent 2 x 2 pictures and certificate of good moral character

High School (Grade 7)

  1. Must have passed the Forbes Academy Entrance Test
  2. Must submit the following: Report Card duly signed by the Principal/Director/President, Certificate of good moral character, NSO Birth Certificate, 2 copies of recent 2 x 2 pictures and brown envelope.

 

SCHOOL FEES

School fees of all programs offered for the entire semester include tuition, laboratory and miscellaneous fees. No additional fees will be charged for the semester.

 

Students may pay in full or in five (5) installments as follows:

1. Enrollment Fee: Php 2,000.00
2. Balance will be divided into four (4) equal installments

AQUENDE SCHOLARSHIP PROGRAM

 
AQUENDE SCHOLARSHIP FOUNDATION, in partnership with FORBES COLLEGE, recognizes the excellent academic performance and community service leadership abilities of graduating high school students by providing scholarship assistance to those who are driven to make a difference in the society. Recognizing the importance of having a college graduate in every Bicolano family, the Aquende Scholarship Program grant partial scholarship subsidies to every high school graduate who has a strong desire to finish college education to be able to help his family and to have a bright future. For school year 2016, each student will receive a tuition fee subsidy of P5,000. It will also provide full scholarships to 100 deserving students. The financial grants and subsidies are also made possible through the initiatives and generous support from one of its mutually-reinforcing institutions, the Center for Global Best Practices Foundation, Inc and the Aquende family.

The Aquende Scholarship Foundation would like to recognize these students, who should be model for the youth and should be active in nation-building for a better Philippines. This is the same principle from which the Aquende Scholarship Foundation has been established. Founded by a young idealistic Albayano, who realized that it is the role of his generation, guided by dedicated teachers, to secure the future of the next generation and of this country. ATTY. HARDY B. AQUENDE, a CPA and a New York esquire, started the foundation with the gifts he received from his wedding in 2013. He pursues scholarship grants for deserving Bicolanos and is an ardent advocate of the youth, education and community development.

Scholarship guidelines and procedures must be observed before a grantee can avail of any of the scholarships offered by the institution.

There are several scholarships available, as follows:

1.1 General Criteria of Eligibility
 
1.1.1 At least high school graduate, and belonging to the top 2 of the graduating class of a minimum of 40 graduates of his/her batch.
1.1.2 Of good moral character
1.1.3 Must comply with the documentary requirements and submit them to the Registrar for evaluation.
 
1.2 Documentary Requirements
 
1.2.1 High School Report Card or Form 138.
1.2.2 Certificate of Good Moral Character from the School
1.2 3 Certification of honor or graduation rank from the Principal.
1.2.4 2 x 2 I.D. pictures (1 pc)
1.2.5 Application for Scholarship
1.2.6 Scholarship Certificate issued by Aquende Scholarship Foundation
 
1.3 Scholarship Privileges
 
1.3.1 An incoming first year student who graduated Valedictorian enjoys full scholarship or 100% Aquende Foundation subsidy of all tuition and fees, the Salutatorian is awarded a 50% Aquende Foundation subsidy of all tuition and fees, and a recipient of Aquende Community Service Leadership Award is entitled to full scholarship or 100% Aquende Foundation subsidy of all tuition and fees.
1.3.2 Entrance scholarship is good for one year or equivalent of two semesters.
1.3.3 This entrance scholarship continues into the following year provided the scholar meets all the requirements of an academic scholar.
 
1.4 Scholar’s Obligations
 
1.4.1 Must carry the regular academic load prescribed by the course.
1.4.2 Must pass all subjects and maintain a GWA of 88% or higher and no grade below 80% in any subject during the duration of the scholarship. In the case of the recipient of the Aquende Scholarship Foundation Community Service Leadership Award, he must pass all subjects and maintain a GWA of 83%.
1.4.3 Must not commit any infraction of the General Rules of Conduct and Discipline.
1.4.4 Attend all the meetings called by the Aquende Scholarship Foundation.
1.4.5 Actively participate in the community service programs of the Aquende Scholarship Foundation and be active members of the Kabataan Para sa Bayan.

2.1 General Criteria of Eligibility
 
2.1.1 Must have obtained a GWA of 88% or higher and with no grade below 80% in any subject of the previous semester. In the case of the recipient of the Aquende Scholarship Foundation Community Service Leadership Award, he must pass all the subjects and obtain a GWA of 83%.
2.1.2 Enrolled in a regular academic load the previous semester.
2.1.3 Must comply with the documentary requirements and submit them to the Registrar for evaluation.
 
2.2 Documentary Requirements
 
2.2.1 Certification of Grades of the previous semester from the Registrar.
2.2.2 Accomplished Aquende Scholarship Application Form for Scholarship from the Registrar’s Office.
2.2.3 Official Notification of the continuing scholarship from the Registrar.
 
2.3 Scholarship Privileges
 
2.3.1 Full scholarship or 100% subsidy by the Aquende Scholarship Foundation of all tuition and fees.
 
2.4 Scholar’s Obligations
 
2.4.1 Must carry the regular academic load prescribed by
the course.
2.4.2 Must pass all subjects and maintain a GWA of 88% or higher and no grade below 80% in any subject during the duration of the scholarship. In the case of the Aquende Scholarship Foundation Community Service Leadership Award, he must pass all subjects and obtain a GWA of 81%.
2.4.3 Must not commit any infraction of the General Rules of Conduct and Discipline.
2.4.4 Must be able to finish the course within the prescribed period.
2.4.5 A maximum of 2 dropped subjects can be considered.
2.4.6 Attend all the meetings called by the Aquende Scholarship Foundation.
2.4.7 Actively participate in the community service programs of the Aquende Scholarship Foundation and be active members of the Kabataan Para sa Bayan.

3.1 General Criteria of Eligibility
 
3.1.1 At least high school graduate
3.1.2 Of good moral character
3.1.3 Must comply with the documentary requirements and submit them to the Registrar for evaluation.
 
3.2 Documentary Requirements
 
3.2.1 High School Card or Form 138
3.2.2 Certificate of Good Moral Character from the School
3.2.3 2 x 2 I.D. Pictures, 1 pc
3.2.4 Application for Scholarship
3.2.5 Result of Entrance Exams
3.2.6 Aquende Foundation Scholarship Voucher
 
3.3 Procedure of Application
 
3.3.1 Submit scholarship application to the Registrar.
3.3.2 Pay the Exam Fee of Php 50.00 to the Cashier.
3.3.3 Take the Qualifying Exam/ Scholarship Exam and wait for the result.
3.3.4 Interview of the Dean or VP for Academic Operations and inform the applicant of the date of the final interview with applicant’s parent/s to be conducted by the President of the Aquende Scholarship Foundation. Scholarship voucher will be issued only during the final interview which will be presented to the Cashier during enrollment. Scholarship voucher is not an enrollment fee. Parent/s are required to attend the final interview together with the beneficiaries.
 
3.4 Scholarship Privileges
 
3.4.1 Partial scholarship as indicated in the scholarship voucher.
3.4.2 This partial scholarship continues into the succeeding semester provided the recipient meets the requirements set forth by the Aquende Scholarship Foundation.
 
3.5 Scholar’s Obligations
 
3.5.1 Must carry the regular academic load prescribed by the course.
3.5.2 Must pass all subjects.
3.5.3 Must not commit any infraction of the General Rules of Conduct and Discipline.
3.5.4 Must be able to finish the course within the prescribed period.
3.5.5 A maximum of 2 dropped subjects can be considered.
3.5.6 Apply for the renewal of the scholarship grant every semester.
3.5.7 Attend all the meetings called by the Aquende Scholarship Foundation.
3.5.8 Actively participate in the community service programs of the Aquende Scholarship Foundation and be active members of the Kabataan Para sa Bayan.
 
3.6 Documentary Requirements for the Renewal of Scholarship Grants
 
3.6.1 Certification of Grades from the Registrar
3.6.2 Certificate of Enrollment of the succeeding semester from the Registrar.
3.6.3 Photocopy of the accomplished registration form of the current semester.
 
IMPORTANT NOTES:
 
1. All scholarships are not applicable to second coursers.
2. All incoming students, freshmen and transferees can apply for the Aquende General Scholarship Program.
